Borggreve-type rotationplasty

Borggreve-type rotationplasty is a surgical procedure that involves:

  • removing the knee and part of the femur,
  • rotating the distal leg segment by 180°,
  • reattaching it so the ankle functions as a knee within a prosthesis.

On paper, the concept is clear. In prosthetic fitting, it’s rarely that simple.

What it really means for prosthetic fitting

1. Hard-to-standardize anatomy

  • Asymmetrical, irregular volumes
  • Malleoli, heel and plantar arch located in unusual load-bearing areas
  • Bony prominences very close to the skin surface
  • Skin often fragile (major surgery, possible radiotherapy)

2. Joint axis ≠ mechanical axis

  • The ankle axis is not naturally positioned where you would want a prosthetic knee axis.
  • There are often offsets between:
    • the anatomical axis,
    • the prosthetic axis,
    • the load line.
  • Alignment is often iterative, with unavoidable trade-offs.

👉 The patient compensates a lot… sometimes too much.
